CenterWell Home Health seeks a Per Diem Home Health Physical Therapist to provide in-home rehabilitative care to seniors and adults with chronic conditions. You will assess functional mobility, develop individualized treatment plans, deliver skilled therapy, and educate patients and caregivers to improve safety and independence at home. Collaborate with an interdisciplinary team, document care in EHR, and manage visits autonomously while upholding our compassionate, patient-first culture.
