COMPENSATION AND BENEFITS Primary Care Physician (FM, IM, BCGM)

Up to $300K+ annual salary
$50K Signing bonus
Up to $25K in annual year-end bonuses
Comprehensive benefits with 28 days of PTO, 5% match on 401K, and $3500 CME allowance
3% annual merit pay increase with quality performance
$20K relocation assistance

RESPONSIBILITIES AND FACILITY DETAILS Primary Care Physician (FM, IM, BCGM)

100% Outpatient - primary focus on Geriatrics patient-population
Preventative care focus on Chronic conditions to reduce hospitalization
Well trusted, Physician-lead healthcare org. with over 120+ locations in 15 states
Patient-focused, small patient panel - 450 patients max
Monday - Friday shift-schedule; No Weekend, No Call
Dedicated care team for full PCP support, as well as Onsite lab, Pharmacy, and X-Ray
