Social Work Care Coordinator

VNS Health seeks a Social Work Care Coordinator to support patients receiving home and community-based care across New York City. In this role, you’ll assess psychosocial needs, create person-centered care plans, coordinate services, and connect patients and families with community resources. You’ll collaborate closely with nurses, therapists, and physicians to ensure continuity of care, monitor progress, and advocate for patient needs. This position requires strong documentation, crisis intervention, and communication skills, with a focus on culturally sensitive, trauma-informed, and compassionate care.
