Full job description

Intermountain Health is seeking a compassionate Registered Nurse Case Manager to provide comprehensive hospice care across our service area. In this field-based role, you will manage a panel of adult and geriatric patients, completing holistic assessments, creating individualized care plans, and leading pain and symptom management to promote comfort and dignity at end of life. You will coordinate closely with physicians, social workers, chaplains, and therapists, educate and support families, document care in the EMR, and contribute to quality and process-improvement initiatives in a mission-driven, team-focused environment.
