Full job description
CenterWell Home Health, part of Humana Inc., seeks a PRN Occupational Therapist to provide compassionate, in-home therapy for seniors and adults with chronic conditions. In this field-based role, you’ll complete evaluations, create individualized care plans, and deliver evidence-based interventions to improve safety, function, and independence at home. You’ll collaborate with an interdisciplinary team, document care using EMR, and educate patients and caregivers.
