Job Description
Job Summary
The Care Manager at Shriners Hospitals for Children is responsible for coordinating and facilitating comprehensive, patient-centered care for children with complex medical needs, ensuring optimal outcomes and a seamless healthcare experience for patients and families.
Key Responsibilities
Perform comprehensive assessments to identify patient and family needs, early in the care process, to ensure smooth progression throughout the continuum of care.
Identify logistical needs, including travel, housing, and other non-clinical resources to facilitate access to care and refer to appropriate support staff.
Develop individualized, goal driven care plans in collaboration with the interdisciplinary team to optimize patient outcomes.
Balance available resources with patient preferences, to facilitate informed decision making.
Utilizes knowledge of Medical Necessity, quality of care, and desired outcomes to support appropriate resource management.
Support interdisciplinary team education and training.
Required Qualifications
RN license in state of practice
Basic Life Support (BLS) Required within 6 months of hire
Preferred Qualfications
1-2 Acute Care/Pediatrics/Specialty Case Management Preferred
Knowledge of care coordination models and regulatory guidelines (e.g., CMS, URAC)
CCM, ACM
BSN
