Registered Nurse Care Manager

Humana seeks an RN Care Manager to coordinate personalized care for members in the La Crosse/Sparta region. In this role, you’ll perform comprehensive nursing assessments, create and manage care plans, and support members across the continuum of care. You’ll collaborate closely with physicians, pharmacists, and social workers, leveraging data and clinical guidelines to close gaps in care and improve outcomes.
