BLS Certification Required Education

Graduate of an Accredited Physical Therapy Program Degree in Physical Therapy (DPT or PT) Must-Have Experience

Minimum

1 year of Home Health Physical Therapy experience HomeCare HomeBase (HCHB) EMR experience required OASIS-E assessment experience required Experience with:

Chronic Disease Management Orthopedic Rehabilitation Total Hip and Knee Replacement Recovery Fall Prevention Balance Training Gait Training CHF Management COPD Management Stroke Rehabilitation Geriatric Patient Care

Preferred Experience

Experienced Home Health traveler Strong care coordination and communication skills Ability to work independently in the field
