Occupational Therapist Home Health

CenterWell Home Health, part of Humana Inc., is seeking an Occupational Therapist to deliver compassionate in-home care to seniors and adults with chronic conditions. In this role, you will perform comprehensive evaluations, develop personalized treatment plans, and provide hands-on therapy to improve safety, function, and independence at home. You’ll collaborate with an interdisciplinary team, educate patients and caregivers, and document care in our EMR.
