Home Health Baylor Physical Therapist

CenterWell Home Health is seeking a Baylor Home Health Physical Therapist to provide compassionate, weekend-focused in-home therapy to adult and senior patients. In this role, you will evaluate functional status, develop individualized care plans, and deliver evidence-based interventions to support recovery, mobility, and independence at home. You’ll collaborate closely with an interdisciplinary team, document care in our EMR, and educate patients and caregivers.
