Responsibilities:



JOB SUMMARY

The Pharmacy Medication Reconciliation Technician will improve medication safety by assisting pharmacists in collection and documentation of up-to-date medication lists for patients being admitted to the organizations hospitals in order to optimize the medication reconciliation process and reduce the number of medication errors secondary to inaccurate or incomplete information.

ESSENTIAL FUNCTIONS
  • Responsible for obtaining and verifying medication history in the electronic medical record in an accurate and timely manner in compliance with standard procedures and regulatory standards.
  • Responsible for accurately documenting medication lists into the electronic health record, communicating to the verifying pharmacist, and relaying any outstanding transition of care issues to the pharmacist and/or provider in a timely manner.
  • Performs other duties as assigned.

EDUCATION
  • High School Diploma or GED Required
  • Other / Certificate Completion of a formalized Technician Training Program. Preferred

EXPERIENCE
  • >10 months pharmacy technician experience or completion of a formalized technician training program. Required
  • Retail pharmacy experience and/or excellent customer service skills Preferred

LICENSE AND CERTIFICATIONS
  • Certified Pharmacy Technician (CPhT) - Certification Current license by AZ State Board of Pharmacy. Required
  • Certified Pharmacy Technician (CPhT) - Certification Pharmacy Technician Certification Board (PTCB) Current Pharmacy Technician Certification (CPhT) issued by Pharmacy Technician Certification Board (PTCB). Required
  • Pharmacy Technician (LPT) - License Pharmacy Technician Diploma Preferred
