CenterWell Home Health, part of Humana Inc., is seeking a Nurse Practitioner / Physician Assistant to provide compassionate, in-home care to seniors and adults with chronic conditions. In this role, you will conduct comprehensive assessments, manage acute and chronic illnesses, and develop individualized care plans in collaboration with an interdisciplinary team. You’ll perform in-home visits, order and interpret diagnostic tests, adjust medications, and educate patients and families on disease management.
