Home Health Physical Therapist | Hospital Affiliated | PSLF Eligible | $5K Sign-On

Highlights/Benefits
  • Hospital-Affiliated Home Health (100% Non-Profit System)
  • Automatically qualifies for Public Service Loan Forgiveness (PSLF)
  • Compensation: $93,000 - $99,000 Base Salary (Based on experience)
  • Pay-Per-Visit (PPV) bonus structure: Earn additional premium pay for any visits completed over the 30-unit weekly target
  • $5,000 Sign-On Bonus
  • Standard $0.63/mile reimbursement for travel (excluding first and last drive)
  • Schedule Control: Dedicated team schedules your start-of-cares, allowing you to route the rest of your day efficiently
  • Caseload: Post-surgical, total joints, COPD, debility, and general medical
  • Weekend on-call rotation (1 weekend per month requirement)
  • Paid Time Off (PTO) = 18 days annually

  • Full-time, permanent position

Physical Therapist Qualifications & Requirements:
  • Bachelor's, Master's, or Doctorate of Physical Therapy from an accredited program
  • Minimum 1-year of inpatient or home health experience required
  • Ability to independently manage your schedule and a metric-driven caseload
  • Team player who collaborates well across disciplines
  • Servant leader mindset
