JOB DESCRIPTION
Adventist Health Ukiah Valley has been deeply connected to the community we serve since 1956. We are comprised of a 50-bed acute care medical center, level IV trauma center, level II intensive care nursery services, extensive physical rehabilitation program, outpatient surgery and diagnostic center and advanced wound care center. Ukiah is a unique small town that charms you with its rich character, arts, vineyards and natural surroundings. Ukiah beckons you to explore all that the valley has to offer amidst our 300 days of sunshine each year. We are also conveniently located just two hours north of San Francisco and 45 minutes from the Pacific Coast.
Up to $20,000 hiring incentive and $5,000 relocation available for eligible rehires and external hires who meet the required qualifications and conditions for payment.
Job Summary:
Plans and conducts segments of treatment programs designed to restore and improve physical, social and mental functions, while meeting department objectives. Serves as co-leader in interdisciplinary approach to patient groups.

Job Requirements:
Education and Work Experience:
- Doctor of Physical Therapy (DPT) degree: Required; only if obtained license on or after January 2016; or foreign equivalent.
- Two years‘ Physical Therapy experience: Preferred
Licenses/Certifications:
- Physical Therapy licensure in state of practice: Required
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy: Required
Facility Specific License/Certifications:
- Basic Life Support (BLS) certification from approved vendor per AH policy: Required
Essential Functions:
- Completes evaluations to ensure quality standards and addresses assessment areas that require attention. Uses appropriate clinical techniques and tools and establishes individualized treatment plans for patients.
- Provides therapy interventions using appropriate treatment protocols and techniques. Establishes individualized care and discharge plans and ensures plans are communicated to appropriate parties.
- Completes comprehensive and accurate clinical documentation within established deadlines.
- Supervises assigned assistants.
- Performs other job-related duties as assigned.
