OCCUPATIONAL THERAPIST

CenterWell Home Health seeks an Occupational Therapist to provide compassionate, in-home therapy services that enhance patient safety, independence, and quality of life. In this field-based role, you will perform comprehensive assessments, design and implement individualized treatment plans, and deliver evidence-based interventions to support activities of daily living and home safety. You will collaborate closely with an interdisciplinary team, maintain accurate, timely documentation, and educate patients and caregivers.
