Full job description

Summary:
The Discharge Support Specialist is responsible for performing administrative and operational functions required in the Care Management Department to support regulatory requirements and patient throughput activities to ultimately ensure patients receive the treatment and services they require post-discharge and are informed of their rights and choice options during the process. The Discharge Support Specialist supports the oversight of the administrative operations of the Care Management Department and acts as the administrative liaison between all clinical departments, community resources, and third-party payers/insurance companies in support of the work of the department.

Experience:
Essential:
1-3 years

Pay Range: $17.00 to $24.75 Based On Experience
