Clinical Nurse Practitioner Educator

HarmonyCares seeks a Nurse Practitioner Trainer in New York to support its home-based primary and palliative care teams. You will onboard and coach NPs on in-home assessments, chronic disease management, telehealth workflows, documentation, and interdisciplinary collaboration. Responsibilities include ride-along visits, skills checkoffs, case reviews, and ongoing education that reinforces quality, safety, and patient-first standards.
