CenterWell Home Health seeks a compassionate Nurse Practitioner or Physician Assistant to provide high-quality in-home care. In this role, you will perform comprehensive patient assessments, develop and manage care plans, prescribe and adjust treatments, and coordinate with an interdisciplinary team to optimize patient outcomes. You’ll educate patients and families, promote autonomy and safety, and ensure continuity of care in the home setting.
