CenterWell Home Health seeks a Baylor-based Home Health Physical Therapist to provide compassionate, in-home care to seniors and adults with chronic conditions. In this role, you’ll perform comprehensive assessments, design and deliver evidence-based therapy plans, and help patients regain mobility and independence at home. You’ll collaborate closely with an interdisciplinary team, document care in our electronic systems, and educate patients and caregivers.
