Family Nurse Practitioner

Elevance Health seeks a Family Nurse Practitioner to provide comprehensive primary and preventive care to diverse member populations. In this role, you’ll perform advanced assessments, diagnose and manage acute and chronic conditions, order and interpret tests, prescribe medications, and develop whole-person care plans. You’ll deliver care in a hybrid model (in-person and virtual), collaborate closely with multidisciplinary teams, and use EHR and telehealth tools to coordinate services.
