Full job description
We're hiring a licensed pharmacist for a contract position where you’ll use your clinical expertise to ensure the safe and thoughtful distribution of medications to individuals navigating a range of physical and mental health challenges.
Minimum Requirements:
- Must have State License to Apply
- EPIC experience
- Experience with medication reconciliation for discharge medication lists
- Communication skills for gathering prior to admission home medication lists from patients, families, or pharmacies
- Communication skills for discharge counseling with patients and families
- Clinical knowledge of antimicrobial stewardship
- Experience interpreting culture results for recommendations
- Knowledge of UTI, pneumonia, and SSTI for recommendations
- Experience with pharmacokinetic dosing management and monitoring
- Experience with anticoagulation dosing management and monitoring
