Position Summary:
We are seeking a compassionate and skilled Registered Nurse (RN) to join our HomeCare team in Baltimore County, MD. This position involves providing in-home nursing care to patients, ensuring high-quality, family-centered services. As a key member of our interdisciplinary care team, you will conduct patient assessments, develop individualized care plans, and collaborate closely with patients, families, and physicians to promote positive health outcomes.
Essential Functions:

Perform initial and ongoing patient assessments in the home setting.

Complete thorough and accurate OASIS documentation.

Develop and implement individualized care plans based on patient and family needs.
-
Deliver professional, safe, and effective nursing care according to established standards.

Educate patients and caregivers throughout the care process and during discharge planning.
-
Coordinate with community resources and other healthcare professionals to optimize care.

Supervise and provide guidance to Licensed Practical Nurses (LPNs) and Home Health Aides.

Document patient progress and revise care plans as needed to achieve desired outcomes.
Qualifications:

Current Registered Nurse (RN) licensure in the State of Maryland.

American Heart Association BLS CPR certification (for healthcare providers).
-
Valid driver’s license in good standing and reliable transportation.

Graduate of an accredited School of Nursing.

Minimum of 1 year of medical-surgical (med-surg) experience preferred.

Strong clinical assessment and critical thinking skills.
-
Experience with OASIS documentation a plus.
