RN Case Manager

As an RN Case Manager at Wesley Woodlawn Hospital & ER, you will coordinate care for hospitalized patients, ensuring safe, timely transitions and optimal outcomes. You will collaborate with physicians, nurses, social workers and ancillary teams to assess needs, develop and implement care plans, arrange post-acute services and advocate for patients and families. Responsibilities include utilization review, discharge planning, education and documentation in alignment with hospital policies and regulatory standards.
