Hospice Medical Director

Heartland Hospice, part of ProMedica, seeks a Hospice Medical Director to lead our interdisciplinary hospice team. The Medical Director provides clinical leadership, oversees patient care plans, and ensures high-quality, patient-centered end-of-life care. Responsibilities include conducting face-to-face visits, certifying and recertifying hospice eligibility, guiding pain and symptom management, collaborating with nurses, social workers, chaplains, and aides, and ensuring regulatory and quality compliance. The role requires strong communication, compassionate decision-making, and commitment to our mission of dignity, comfort, and support for patients and families.
