Full job description
Minimum Requirements
* Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
* Active State License is Required to Start the Assignment
* BLS Certification May Be Required from AHA or ARC
* Home health experience
* Experience with EMAR system HCHB
* PT Practice 12365
Assignment Details
* Schedule: Monday through Friday, 8:00 AM to 4:30 PM
* Weekend and on-call requirement: None
* Coverage areas: Marion and Hamilton
* Mileage reimbursed between visits from first patient to last patient
* Responsibilities include resumption of care, start of care, discharges, and oversight of PTA
* Productivity expectation: 30 points
