ROLE AND RESPONSIBILITIES:

COORDINATION OF CARE
Facilitate access to various community resources to ensure clients’ basic needs are met.
Offer referrals to Case Management for treatment services when necessary.
Collaborate with case managers and treatment staff to address mutual client needs.
CASEWORK ACTIVITIES
Offer guidance on the recovery experience, including stage-appropriate recovery education.
Offer support and advocacy related to obstacles that are often encountered during the early stages of recovery.
Maintain a caseload of clients and assist with case coordination needed for client needs.
Assist in the development of individualized recovery plans for participants.
Assist participants in acquiring self-care skills such as budgeting, shopping, cooking, housekeeping, interview skills, etc., through role plays and/or direct assistance.
Monitor participants' recovery stability.
Monitor the degree to which the current service plan meets the participants' recovery needs.
Introduce and engage participants in the recovery community.
Provide ongoing assistance for participants in obtaining and/or maintaining services from multiple systems.
Transport or arrange for transportation for participants to treatment or other recovery support appointments/activities.
Assist case management staff with outreach activities to engage new participants in the recovery process.
Actively participate in activities provided in The Living Room to further engage individuals in the recovery community.
AFTERCARE
Perform follow-up activities appropriate for participants' aftercare plans until participants' recovery has stabilized.
Serve as a liaison to program participants for re-admission recovery concerns.
Provide ongoing support to participants who have completed the program, as needed.
OTHER DUTIES
Attend and participate in staff meetings, case conference and other job-related meetings, clinical functions and/or activities.
Attend all supervision activities initiated by the Case Management Supervisor. Solicits supervision about all issues which necessitate supervisory input/feedback.
Perform other tasks, duties, or functions relating to the achievement of overall goals & objectives of the Executive Commission.
Adhere to Agency’s Ethical Code of Conduct.
Comply with the federal and state regulations on the confidentiality of alcohol and drug abuse patients as described in 45 CFR, Part 160 and 164, 42 CFR Part 2, Act 33, and Act 126.
Other duties as assigned by administrative staff.
MINIMUM EDUCATION/REQUIREMENTS:
Maintain a minimum of 2 years of self-sustained recovery outside of a restrictive setting.
Certified Recovery Specialist (CRS) or Certified Family Recovery Specialist (CFRS) certification through the Pennsylvania Certification Board.
Valid PA Driver’s License and active automobile insurance.
Ability to obtain a satisfactory Pennsylvania State Police Criminal History check, a DHS (Department of Human Services) Child Abuse History Certification, and FBI Fingerprint-based Criminal Background Check.
No felony convictions in the last 5 years.
Must pass pre-employment drug screen. xqrzhdi
