Physical Therapist Assistant (PTA) – Skilled Nursing Facility (SNF)
We are seeking a dedicated and skilled Physical Therapist Assistant (PTA) to join a collaborative therapy team within a skilled nursing facility. This role focuses on providing high-quality rehabilitation services to short-term rehab patients and long-term residents, helping them regain mobility, independence, and confidence in daily activities.
The PTA will implement individualized treatment plans developed by the supervising Physical Therapist, delivering skilled interventions that improve strength, balance, gait, and functional mobility. This position requires strong clinical judgment, communication skills, and the ability to manage a productive caseload in a supportive, team-based environment.
Key Responsibilities:
Provide skilled physical therapy interventions under the supervision of a licensed PTAssist patients with transfers, gait training, balance, strength, and endurance exercisesImplement fall prevention strategies and assist with adaptive equipment recommendationsEducate patients and caregivers on safe mobility and home exercise programsMonitor patient progress and communicate updates to supervising PTAccurately document treatment sessions, progress notes, and discharge summaries in EMRCollaborate with interdisciplinary team members including OT, SLP, nursing, and case managementMaintain productivity while ensuring patient-centered careIdeal Candidate:
Active PTA license in practicing stateSNF or geriatric rehabilitation experience preferredStrong clinical reasoning and communication skillsComfortable managing both short-term rehab and long-term resident caseloadsThis is an excellent opportunity for a PTA who is passionate about geriatric rehabilitation and making a meaningful impact on patient functional outcomes in a supportive, team-oriented setting.Towne U.S. LLC Job ID # .
