Minimum Requirements:
- Bachelor‘s, Master‘s, or Doctoral Degree in Physical Therapy from an accredited school approved by the APTA.
- Current Wisconsin Physical Therapist (PT) license.
- Experience working with adult and geriatric populations in skilled nursing or long-term care settings is preferred.
- Experience with documentation and compliance; experience with Net Health is a plus.
- BLS certification from the American Heart Association or American Red Cross may be required.
- Strong communication, teamwork, and professional conduct.

Your Responsibilities
- Provide compassionate and effective physical therapy services to adult and geriatric residents in a skilled nursing facility.
- Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence.
- Support residents in achieving personal goals and rehabilitation milestones.
- Accurately document care and maintain compliance standards.
- Collaborate with therapy and nursing teams to ensure patient-centered outcomes.
- Educate residents and families on safe techniques and strategies to maximize independence.
- Participate in team meetings and contribute to care planning.
