TriStar Health seeks a compassionate Rehab Nurse to deliver evidence-based rehabilitative care to adult patients in our Healthcare & Medical Services department. In this full-time RN role, you will assess patients, develop and manage individualized care plans, coordinate with an interdisciplinary rehab team, and support mobility, ADLs, and safe discharges. You’ll document care in the EHR, educate patients and families, and champion quality and safety.
