Full job description

PURPOSE

The Licensed Clinical Social Worker (LCSW) provides advanced psychosocial support, counseling, and care coordination services to patients and families in inpatient, outpatient, and ambulatory care settings. As a key member of an interdisciplinary team, the LCSW delivers evidence-based interventions, facilitates access to healthcare and community resources, and supports patients in navigating complex psychosocial challenges to improve overall health and well-being. This role is critical in addressing social drivers of health, developing individualized care plans, and contributing to effective discharge planning and crisis intervention. The LCSW also plays an integral part in program development, quality improvement initiatives, and mentoring of social work team members. The LCSW collaborates closely with physicians, nurses, case managers, and allied health professionals to ensure a holistic, patient-centered approach to care. This position requires independent clinical practice within the scope of licensure, as defined by the Texas State Board of Social Work Examiners (TSBSWE), and adheres to high standards of ethical and professional conduct.

RESPONSIBILITIES

Clinical Assessment, Psychotherapy, and Intervention

  • Conduct comprehensive biopsychosocial-spiritual assessments and provide evidence-based psychotherapy using modalities such as cognitive behavioral therapy, motivational interviewing, and supportive therapy.
  • Deliver psychosocial education and crisis intervention services to patients and their support networks.
  • Document interventions in compliance with organizational and regulatory standards.

Care Coordination and Community Resource Navigation

  • Facilitate access to community resources and support services to address social drivers of health.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans and support transitions of care, including discharge planning.

Program Development and Quality Improvement

  • Contribute to the design and implementation of social work programs, participate in quality improvement initiatives, and support data collection and reporting to improve patient outcomes and care delivery processes.

Mentorship and Professional Development

  • Provide clinical guidance and support to LMSWs and social work interns, fostering professional growth and adherence to best practices in clinical social work.
  • Engage in ongoing professional development to maintain licensure and current knowledge of evidence-based practices.

Team Engagement and Coverage Support

  • Participate in care team meetings, rounds, case conferences, interdisciplinary discussions, and organizational initiatives.
  • Provide coverage for social work colleagues during absences or high-volume periods to ensure continuity of care.

REQUIRED QUALIFICATIONS:

  • Master's Degree in Social Work (MSW) from a CSWE-accredited program
  • At least 2 year(s) of experience in post-degree clinical experience in a healthcare setting
  • Current licensure as a Licensed Clinical Social Worker (LCSW) in the State of Texas.

Relevant education and experience may be substituted as appropriate:

PREFERRED QUALIFICATIONS:

  • At least 3 year(s) of experience in post-degree clinical experience in a healthcare setting.
  • Experienced in conducting assessments and delivering evidence-based treatments, with working knowledge of brief interventions, motivational interviewing, and healthcare delivery.
  • Bilingual in English and Spanish.
  • Basic Life Support (BLS) certification

SALARY RANGE

$70,000 + depending on qualifications

WORKING ENVIRONMENT/EQUIPMENT

  • Standard office equipment and clinic environment.
  • Repetitive use of a keyboard and computer systems.
  • May be exposed to such occupational hazards as communicable diseases, bloodborne pathogens, ionizing and non-ionizing radiation, hazardous medications, and disoriented or combative patients, or others.
