Primary Care Physician

Oak Street Health is seeking a compassionate Primary Care Physician to deliver high-quality, preventative care to an adult Medicare population in underserved communities. You will manage a dedicated panel of patients, perform comprehensive primary care visits, coordinate chronic disease management, and lead care planning in collaboration with nurses, care managers, and social workers.
