Licensed Practical Nurse (LPN)

Trinity Health IHA Medical Group is seeking a compassionate and motivated Licensed Practical Nurse to support our providers in delivering exceptional patient care. In this role, you will prepare patients for a provider visit, provide results management support, insurance company advocacy, assist with clinical procedures, support care coordination and gaps in care and medication/vaccine administration in order to help create a positive healthcare experience every day.

 

What You'll Do:

• Welcome and prepare patients for appointments.
• Determines and documents reason for the visit.
• Uses critical thinking skills within the scope of the role to assess if patient information requires higher level follow up.
• Collect and document patient histories, vital signs, and health information.
• Assist providers with exams, procedures, testing, and specimen collection.
• Administer medications and perform diagnostic testing as directed.
• Communicate test results and provider instructions to patients.
• Performs general specimen collection duties.
• Provide patient and family education to support better health outcomes.
• Coordinate care and serve as a liaison between patients and providers.
• Support quality improvement initiatives and disease management programs.

 

General Requirements:

 

EDUCATION: High School Diploma or GED and graduate of an accredited Licensed Practical Nurse Program. 

 

CREDENTIALS/LICENSURE:  Valid, unrestricted LPN license in State of Michigan.  Valid CPR certification.

 

MINIMUM EXPERIENCE:  1-2 years of clinical experience in a medical or physician office.  Previous nursing experience not required.
