Nurse Practitioner

Legacy Health is seeking a Nurse Practitioner to provide advanced primary and specialty care in an outpatient clinic setting. In this role, you will assess, diagnose, and manage acute and chronic conditions, order and interpret diagnostic tests, prescribe medications, and develop evidence-based care plans. You will collaborate closely with physicians, nurses, and interdisciplinary teams to deliver patient-centered, culturally competent care. Responsibilities include patient education, care coordination, documentation in the EHR, quality and safety initiatives, and contributing to a supportive, learning-focused clinic environment that reflects Legacy’s community health mission.
