Family Medicine NP/PA

Community Care Physicians seeks a Family Medicine NP/PA for our Slingerlands practice. In this outpatient role, you’ll provide comprehensive primary care across the lifespan, including acute visits, chronic disease management, preventive care, and patient education. You’ll work within a collaborative, physician-owned multispecialty group that values patient-centered care, quality improvement, and work–life balance.
