Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Texas Physical Therapy license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Home Health experience required, preferably geriatric home health setting.
- Experience with home health EMR systems. Experience with HCHB is preferred.
- Reliable transportation with expected mileage approximately 250 miles per week.
- Ability to cover territory including Tyler, Longview, and Marshall. Floating to other nearby locations may be required.
- Weekend rotation: No. On call: No.
Assignment Details
- Facility Type: Home Health.
- Shift Name: Days.
- Shift Schedule: 5 days per week.
- Caseload Focus: Geriatric home health patients.
- Territory/Coverage Area: Tyler, Longview, Marshall.
- Expected Mileage Per Week: Approximately 250 miles.
- Weekend Rotation: No.
- On Call: No.
- EMR: HCHB preferred.
