CenterWell Home Health seeks a compassionate Occupational Therapist to provide per diem in-home therapy for adult and senior patients. In this role, you will evaluate functional abilities, develop and implement individualized treatment plans, and deliver evidence-based interventions to improve safety, mobility, and independence at home. Collaborate with an interdisciplinary team, educate patients and caregivers, document care in EMR, and support patient-centered goals while upholding high standards of clinical quality and professional ethics.
