Responsibilities:

• Provide direct patient care, including vital sign monitoring, wound care, and medication administration
• Assess patient needs and develop individualized care plans in collaboration with the healthcare team
• Implement and evaluate patient care plans, making adjustments as necessary

Requirements:

• Licensed Practical Nurse (LPN) with current licensure in the state of New York
• High school diploma or equivalent required; degree in nursing or related field preferred
• Minimum 1 years of experience in a skilled nursing setting
