Physical Therapist

CarePartners is seeking a compassionate Physical Therapist to provide one-on-one, in-home rehabilitation services to adult and geriatric patients. In this role, you will perform comprehensive assessments, design individualized treatment plans, and deliver evidence-based interventions to improve mobility, safety, and independence. You will collaborate closely with an interdisciplinary home healthcare team while documenting care in our electronic record system.
