In-Home Health Physician Assistant

Signify Health is seeking a Physician Assistant to perform comprehensive in-home health evaluations for diverse patient populations. You will obtain medical histories, perform physical exams, reconcile medications, and document findings in our digital platforms to support value-based care. Collaborating with interdisciplinary teams, you’ll identify care gaps, educate patients, and coordinate follow-up services.
