Minimum 1 year of recent experience in outpatient physical therapy Active PT license required at time of consideration License required if applicable No certifications expiring within the first 30 days of assignment unless renewal documentation is provided Valid U.S. Social Security Number required No employment gaps of 90+ days within the past 12 months Additional Information

Up to 5 days of requested time off allowed, must be made up during the assignment Local candidates accepted, though a radius rule may apply to bill rate and contract eligibility Candidates residing within 50 miles of the facility are limited to 3 consecutive 13-week contracts before requiring a 1-year separation or transfer outside the radius
