Physician Assistant

Signify Health seeks a Physician Assistant to deliver value-based care through in-home and virtual assessments. You will perform comprehensive health evaluations, review medical histories, reconcile medications, order/interpret basic tests, and document accurately in the EHR. Partner with interdisciplinary teams to close care gaps, coordinate follow-up, and support quality and risk adjustment programs. Ideal candidates bring strong clinical judgment, autonomy in the field, excellent communication skills, and a passion for improving outcomes for vulnerable and Medicare/Medicaid populations.
