Physical Therapist, Home Care

VNS Health seeks a dedicated Physical Therapist for home care in New York City. In this role, you will perform comprehensive evaluations, design individualized treatment plans, and deliver evidence-based interventions to help adult and geriatric patients regain mobility and independence at home. You will educate patients and caregivers, document care in the EMR, and collaborate closely with nurses, physicians, and other team members.
