CenterWell Home Health seeks a compassionate Home Health Physical Therapist to deliver one-on-one, evidence-based rehabilitation in patients’ homes. You’ll complete evaluations, design and implement individualized treatment plans, and focus on improving mobility, safety, and independence for adult and geriatric patients. Collaborating closely with an interdisciplinary team, you’ll document care thoroughly, educate patients and caregivers, and support quality and safety initiatives in a culture defined by compassion, integrity, and clinical excellence.
