In-Home and Telehealth Physician Assistant

Signify Health seeks a mission-driven Physician Assistant to perform in-home and virtual clinical evaluations for diverse patient populations. You’ll conduct comprehensive health assessments, manage chronic and acute conditions, and document findings in EHR systems to support value-based care and risk adjustment.
