Minimum Requirements
- Master's or Doctoral Degree in Physical Therapy from an accredited program.
- Active Texas Physical Therapist license required prior to start.
- BLS certification from AHA or ARC may be required.
- Home health and HCHB experience required.
- Experience with home health documentation: SOCs, ROCs, recertifications/reassessments, and plan of care development.
- Experience supervising PTAs.
- Preferred: orthopedics and neuro experience.
Assignment Details
- Facility Type: Home Health.
- Shift Name: Days.
- Shift Details: Monday–Friday, 08:00–16:00.
- Weekly Hours: 40.
- Productivity Expectation: 30 points.
- Coverage Areas: Hawkins, Mineola, Lindale (local travel between patient visits required). Mileage reimbursed between visits from first to last patient.
- Primary responsibilities: patient assessment, complete plan of care for home health, SOCs, ROCs, recerts/reassessments, supervise PTA.
- Dress code: scrubs in black, gray, or purple.
