Position Overview
Heal Motion Care is actively seeking dedicated and compassionate Physical Therapists to join our dynamic team in Chula Vista, California. The role requires on-site presence as we provide home-based care to both children and adults.

Work Environment
The working environment at Heal Motion Care is multifaceted and collaborative. As a Physical Therapist, you will operate primarily in clients' homes, addressing their rehabilitation needs in a familiar and comfortable setting. Key Responsibilities
- Conduct thorough physical therapy evaluations for clients, assessing their functional capabilities and limitations.
- Design and implement individualized treatment plans based on evaluation outcomes, tailoring interventions to the client's specific conditions and goals.
- Regularly monitor and document patient progress, utilizing electronic health record systems to prepare SOAP notes and care plans efficiently.
- Engage with clients and their families, educating them about the recovery process, challenges, and preventive measures to improve health outcomes.
- Collaborate with multidisciplinary teams, including Occupational and Speech Therapists, to ensure comprehensive care for clients receiving multiple therapy services.
- Adhere to company standards by meeting minimum utilization goals and achieving other relevant business objectives.
- Embody and promote the core values of Heal Motion Care, acting as an ambassador for our mission and client care philosophy.
Patient Population / Client Focus
Our Physical Therapists work with a diverse array of clients, including children and adults across various backgrounds. We focus on individuals with unique therapeutic needs, ranging from mobility issues to post-operative recovery, and we strive to provide culturally competent care that acknowledges and respects their diverse histories and experiences.
Qualifications
- The ideal candidate will possess a Master's degree or Doctor of Physical Therapy (DPT) degree from an accredited institution.
- A current, unrestricted license from the Board of Physical Therapy to practice in California is mandatory.
- Ability to perform physical evaluations and treatments both in-home and via telehealth platforms, ensuring accessible care for all clients.
- A passion for serving a wide range of patients, demonstrating adaptability to different age groups, races, and cultural backgrounds.
- Strong collaboration skills, emphasizing teamwork and a client-focused approach to care.
Preferred Experience
- Previous experience working in home health care services is preferred, but not required.
- Familiarity with electronic healthcare documentation and telehealth practices is a plus.

Compensation and Benefits
Heal Motion Care offers a competitive salary range of $100,000 to $120,000 per year, commensurate with experience and qualifications.
