Job Description <br><p><strong>Pay range for this position is $43.90 - 63.65 / hour BOE </strong></p><p><strong>Position Summary</strong></p><p>On a flexible, as-needed basis, provides clinical services to all residents residing at Communities of Christian Living Communities (CLC). These services will include skilled nursing, assisted living, and/or rehabilitation services. </p><p><strong>Rehabilitation Flex Team Members are exempt from a multiple-shift requirement, and are encouraged to cover needs at multiple sites as they are able.</strong></p><p>Diagnoses and treats individuals with speech, language, voice, and fluency needs. This role is grounded in building genuine, trust-based relationships with residents and their families - we believe every person, team member and resident alike, brings unique strengths and perspectives that make our community stronger.</p><p><strong>Essential Duties</strong></p><ul><li>Performs assessments and diagnoses, and develops treatment plans for language and swallowing disorders.</li><li>Evaluates levels of swallowing, speech, and language difficulty and identifies appropriate treatment options.</li><li>Creates and carries out individualized treatment plans that address each resident's specific functional needs and personal goals.</li><li>Teaches residents techniques to improve sound production, voice, and fluency, tailored to what resonates with each individual.</li><li>Creates goals and objectives for residents to work toward, celebrating progress and working collaboratively through challenges.</li><li>Provides support and advice to residents, their caregivers, and family members with warmth and patience.</li><li>Counsels residents and families on ways to cope with swallowing and communication disorders, fostering trust throughout the process.</li><li>Builds strong relationships with residents over time, taking a genuine interest in their personalities, preferences, and everyday lives.</li><li>Compiles thorough notes on residents’ progress and adjusts therapies and treatment plans where necessary.</li><li>Documents resident care services according to approved medical and facility documentation procedures.</li><li>Collaborates closely with fellow team members, contributing to a supportive, inclusive workplace culture where diverse backgrounds and perspectives are welcomed and celebrated.</li></ul> <br>Qualifications <br><p><strong>Basic Qualifications & Experience</strong></p><ul><li>Education: Graduate of an accredited therapy program.</li><li>Experience: 2–3 years of related experience, or an equivalent combination of education and experience.</li><li>Licenses/Certifications: Current Speech Language Pathologist state license; CPR certification.</li><li>Excellent verbal & written communication, active listening, and social perceptiveness</li><li>Relationship building, compassionate instruction, and genuine appreciation for individuality and diverse life experiences</li><li>Critical thinking, sound judgment, and decision-making</li><li>Time management and service orientation</li><li>Collaborative teamwork</li></ul><p><strong>Working Conditions / Physical Requirements </strong></p><ul><li>Physical stamina to lift, transfer, and assist residents through physical activities.</li><li>Requires frequent lifting and carrying of items weighing up to 50 lbs. unassisted, including assisting residents when needed.</li><li>Must be able to communicate clearly with residents in person and over the phone.</li><li>Visual acuity adequate to perform job duties, including visual examination of residents and reading information from printed sources and computer screens.</li></ul> <br>Additional Information <br><p>If you are considering a position at Christian Living Communities | Cappella Living Solutions, we have a wide range of benefits to consider!
