RN-Patient Navigator

Ascension is seeking an RN-Patient Navigator to guide patients and families through the healthcare continuum. In this role, you’ll coordinate care, provide education, and remove barriers to treatment across Ascension sites of care. You will assess patient needs, schedule follow-up services, collaborate with physicians and interdisciplinary teams, and promote adherence to care plans.
