Position Description:
 

Responsible for performing and documenting psychosocial assesments, coordinates educational and outreach services relevant to the social services, and provides liaison function between system-wide outpatient clinics.

Qualifications:
1. Master’s degree in Social Work from an accredited institution
2. Experience in Palliative Care or hospice setting preferred
3. Licensed Clinical Social Worker (LCSW) certification required
4. Strong understanding of end-of-life care and ethical issues in Palliative Care.
5. Minimum of 2 years’ experience in a healthcare setting
6. Familiarity with Epic, ICD-10, and CPT coding preferred.
