PRN Home Health Physical Therapist

Frankfort Regional Healthcare at Home seeks a PRN Home Health Physical Therapist to deliver evidence-based rehabilitation services to adult and geriatric patients in their homes. Responsibilities include performing comprehensive evaluations, developing individualized care plans, providing therapeutic exercises and gait training, and educating patients and caregivers on safety and mobility. The therapist will collaborate closely with nurses, physicians, and the interdisciplinary team to optimize outcomes, document accurately in the EMR, and uphold our patient-centered, compassionate standards of quality, safety, and service excellence.
